I don't know how it works in UK and Sweden; being the past president of italy's ISPs association I had to deal with this in Italy.

for sites operating in italy that host this content, IPS work cooperate with the police for the takedown and user identification. 

for non italian hosts, there's a blacklist of IP address for which italian providers must redirect all traffic to a black hole (for web traffic, a page saying that the site is not reachable for this reason)
 
the point is that most of this infamous content is not hosted on specific dedicated hosts and in case of shared hosting, the IP address blacklist does not work (otherwise one'd block access to legitimate content as well)

so, except for mainstream media and politicians,... we all know it has an extremely limited effect (at most)
